About Us
Continuum Nursing & Care is a privately owned Ontario-based nursing service providing professional, compassionate, and evidence-based care in the community. Our services are delivered by nurses licensed and regulated in Ontario.
We specialize in personalized in-home and virtual nursing services designed to support individuals and families across the continuum of care — from short-term support to ongoing health management. Our approach is client-centred, discreet, and grounded in safety, professionalism, and respect.
Our Services
Continuum Nursing & Care offers a wide range of private-pay nursing and supportive services, including:
• Health assessments and monitoring
• Medication administration
• Wound and skin care
• Catheter care, IV maintenance, ostomy care, G-tube, tracheostomy, and NG tube support
• Palliative and supportive care
• Home care and assisted daily living support
• In-home respite care (all ages)
• Care planning and patient education
• Mental health support and counselling
• Diabetic foot care and general foot care
• Medical provider notes and certificates
• Suture and staple removal
• Urinalysis (UTI screening with medical note)
• Pregnancy testing (urine hCG with medical note)
• Drug screening with medical documentation
• Disease prevention and health promotion
• Safety support and client advocacy
• Medical forms (school, work, volunteer, daycare, and camp forms)
• Medical record printouts (per-page fee applies)

How We Work
We collaborate with clients, families, and — when appropriate — other healthcare providers to support continuity of care and positive health outcomes. Services are available in person within our service area and virtually across Ontario, depending on the service.
Payment & Insurance
All services are offered on a private-pay basis and are not covered by OHIP. Payment is due at the time of service. Detailed receipts are provided and may be submitted to private insurance providers for possible reimbursement.
Insurance coverage and reimbursement are determined solely by the client’s insurer. Continuum Nursing & Care does not guarantee reimbursement.

Accessibility & Equity
Continuum Nursing & Care recognizes that not everyone is able to access private-pay healthcare services. We are committed to exploring responsible ways to improve accessibility while maintaining safe, ethical, and sustainable care delivery.
Under current Ontario regulations, our services are not eligible for billing under OHIP. As an independent provider, we continue to seek opportunities for partnerships, alternative funding pathways, and community-based solutions that may help reduce barriers to care over time.
