Continuum Nursing & Care is a privately owned Ontario nursing service providing professional, compassionate, and evidence-based care in the community. Operating under 14519922 CANADA CORP, our services are delivered by Registered Practical Nurses (RPNs) licensed with the College of Nurses of Ontario.

We specialize in personalized home nursing services designed to support individuals and families across the continuum of care — from post-hospital discharge support to ongoing health monitoring and chronic condition management. Our focus is on safe, client-centred care delivered with professionalism, discretion, and respect.

We work collaboratively with clients, families, and other healthcare providers to promote continuity of care and support optimal health outcomes.

Our services are offered on a private-pay basis and are not covered by OHIP. Payment is due at the time of service. * We provide detailed receipts that you may submit to your insurance provider for reimbursement.
