• Nurse name, credentials, and CNO registration number
  
• Date(s) and duration of service
  
• Description of nursing services provided
  
• Fees paid
  

Insurance coverage and reimbursement are determined solely by the client’s insurance provider. Continuum Nursing & Care does not guarantee reimbursement and is not responsible for denied or partially reimbursed claims. All services are to be paid in full at time of service.
